ABSTRACT
Azygos artery (a. azygos) in anterior cerebral artery A2 segment is a rare anatomic anomaly, as well as giant saccular aneurysm in the A2 segment of a. azygos trunk. Giant aneurysm of the azygos A2 segment artery is incidental and thus far literature reported only a small number of cases, treated surgically after the subarachnoid hemorrhage.
The literature also provides description of coexistence of an azygos artery with corpus callosum hypoplasia, porencephalia or vascular malformation. Angiography may suggest anomaly. Final verification is possible during the surgical preparation and exposure of the vessels in longitudinal fissure of the brain. The azygos A2 trunk is the special situation where the large brain region is vascularized by simple trunk of artery and this vessel has a specific anatomic characteristic. The problem of potential consequences of postoperative ischemia after temporary clipping becomes more prominent during aneurysm surgery.
